Consent is only protective if it is complete, current, and connected to the treatment it covers. A signature on a form in a drawer is not the same as a defensible record.
What complete consent requires
- The correct, current version for the procedure
- A timestamp and an attributable signer
- A link to the specific treatment and provider
- Photo documentation captured with matching consent
